Choices for Independence (CFI)
Choices for Independence (CFI) is a Medicaid waiver program run by the New Hampshire Department of Health and Human Services and is designed to provide home care as an alternative to a skilled nursing facility. Under the CFI program, individuals or couples 62 or older may live at Summercrest provided they meet both the medical and financial criteria of this program.
Summercrest has a limited number of apartments available for Choices For Independence recipients. Interested seniors can join the waiting list for this program and be in line for the next available apartment by applying for residency.

- Personal emergency response system
- Physical therapy, occupational therapy, speech therapy
- Medical services, MD office visits, prescriptions, hospitalization, laboratory services, x-ray services, wheelchair transport, chiropractic and podiatrist services
